10th Biennial Childhood Obesity Conference

Fees

We are pleased to invite you to attend the Childhood Obesity Conference -- the nation’s largest, most influential collaboration of professionals dedicated to combating pediatric obesity.  Full conference registration includes one reception, one breakfast, two continental breakfasts and all conference materials.  Attendee registration is now open!

Early Bird Registration $375 (on or before January 31, 2019)
Student Registration* $260 (on or before January 31, 2019)

$300 (February 1 to on or before May 31, 2019)
Registration $425 (February 1 to on or before May 31, 2019)
Late Registration
*No student rate available during late registration period.
$450 (after May 31, 2019)
 One Day Registration 

$100 (Monday, July 15, 2019) (Pre-Conference sessions are an additional fee)
$225 (Tuesday, July 16, 2019)
$225 (Wednesday, July 17, 2019)
$225 (Thursday, July 18, 2019)
 
Non-profit Exhibitor $600 (Additional charges will apply for electrical and/or internet access.)
Commercial Exhibitor $850 (Additional charges will apply for electrical and/or internet access.)
Poster Session Complimentary to present a poster (you must be a paid registrant at the conference)
Host a Roundtable Complimentary to host a roundtable (you must be a paid registrant at the conference)

 

Payment Policy

Acceptable forms of payment include:
Credit Cards (VISA, Mastercard, Discover)
Purchase Orders (Purchase orders must be uploaded at the time of registration. If you are not able to have a PO issued at the time of registration, please upload a document on letterhead that states a PO has been requested.  Then email the PO to cts_registration@csus.edu once issued)
Checks (Checks must be mailed prior to the Conference start date)

*If you are interested in registering for the conference under the student category, you must submit student verification information. Current (2018-2019) student verification may take the form of a student ID (student ID must clearly indicate current semester/year enrollment), 2018-2019 semester/quarterly/annual class schedule or 2018-2019 report card. After selecting the student rate option on the registration form, you will receive an automatic email confirmation and next steps in order to verify your student status. Your conference registration at the student rate will not be approved until the verification is received.

Registration Agreement

Upon receipt of this registration, Sacramento State and hosting organization consider the registrant formally enrolled for the conference/training. All registrants are responsible for full payment. Payment must be received prior to the conference, with the exception of a Billing Agreement. A Billing Agreement may be made between an organization and Sacramento State whereupon an invoice will be sent to the organization after the conference. Individual attendees may not enter into a Billing Agreement. Failure to submit payment and/or if registrant is a “No Show”, organization will still be responsible for full payment. If the organization declines to pay Sacramento State for any reason, the attendee is responsible for the entire conference/training fee and any costs incurred for collection of fees.

Cancellation Policy

All cancellations must be emailed to cts_registration@csus.edu.  Unfortunately phone messages will not be accepted as a form of cancellation.  Cancellations received by April 30, 2019, and are subject to a $50 fee. After April 30, substitutions will be accepted, but refunds will not be provided. No Shows will be billed the full registration fee.
